DUTY DOCTOR SERVICE – FOR WHEN YOU HAVE AN URGENT MEDICAL PROBLEM

This service offers you quick access to your GP practice, where they know you and have your full medical record. We recommend this service instead of going to A&E at the hospital

  1. Phone the surgery and explain to reception that you have an urgent problem
  2. Reception can then put you on the Duty Doctor list
  3. The Duty Doctor will ring you back within 2 hours
  4. The Duty Doctor will decide with you what you need, which could be:
  • Advice over the phone
  • An appointment at the surgery
  • A home visit if you are really not well
  • A referral to another service

In order to register you need to live within a certain distance of our practice – We accept residents from N16, N4, N5 postal codes living within a certain distance of our practice. Please complete the following fields as fully as possible, this will help us to process your details. We will contact you to acknowledge receipt of your registration. The first time you attend the practice for an appointment we will need to ask you to complete a brief form and provide us with proof of identity.

Please note that we cannot currently accept registrations for children online. You need to be over 16 to use this service. Children are very welcome to register with us, but a parent or guardian needs to visit the practice to complete the form in person.

As a security measure all information provided on this form will be sent as a secure, encrypted email.

The Accessible Information Standard

The Accessible Information Standard is a new law to make sure that people who have a disability, impairment or sensory loss are given information they can easily read or understand.

The Accessible Information Standard tells NHS and adult social care organisations they must make sure people get information in different formats such as: – easy read, braille, advocate, British Sign Language (BSL), large print or email.

 Please inform a member of staff if you have any communication support needs. You need to help us make sure we get things right for you.
